Forms
Choose from our three service options listed below:
- Online Forms: Full-service option on select products.
- Self-Service Forms: Apply online and complete request in-person or by mail.
- Printable Forms: Complete form and submit in-person or by mail.
Online Forms is recommended when you are unable to come into our office and you would like to purchase a birth, death, or marriage certificate. While completing the form you will choose your delivery method and apply payment online. Processing notifications are sent to the email address provided with your request.
Our other online services include:

- Electronic Filing - Form 700
When you are local and planning to come into our office submit your application prior to your visit via the Self-Service Form. Your application will remain in our Self-Service Queue through to your arrival. Payment is collected at the service counter and the record requested provided to you at the time of service.
The following services must be completed in-person:
- Marriage License Issuance
- Both marriage parties must come in together and provide their current government issued photo identification.
- Submit prior marriage Decree of Dissolution if finalized within six months.
- Deputy Commissioner for a Day
Official Record Copy Requests may be completed by calling our office at 530-666-8130 and providing your credit card information for payment. Certified copies must be delivered by mail, regular copies can delivered by email.
Documents and Filings can be completed and submitted in-person or by mail by printable forms.
In-Person Requests:
Our service counters are open Monday - Friday, 8:00 a.m. to 4:00 p.m. (no lunch closure) excluding holidays.
- Real property record documents must be selected, prepared, and fully executed prior to submission. Documents presented for recording must be submitted with sufficient time to examine; therefore, it is recommended you arrive before 3:30 p.m. to ensure same day recording of your document.
Mail-in Requests:
All requests received by mail are processed within 1 to 3 business days of receiving.
